Request for Confirmation of Completion of PDEP-Accredited Academic Program Form

This form is intended for Brescia/Western students who have completed, or are currently completing their final year in the Honours Specialization in Nutrition and Dietetics program, to request a signed copy of their ‘Confirmation of Completion of PDEP-Accredited Academic Program Form’ by the Academic Director. This form will be needed for your dietetic practicum program applications.

Please ensure you submit your form request between October 27 - November 17, 2023.
Students who meet this deadline will receive their signed confirmation form back via email between December 4 - 20, 2023.
